Hazel Arilla Jewkes Blackham Wayman, 94, beloved mother, grandmother and great-grandmother, passed away peacefully of causes incident to age, July 5, 1990.

Born July 27, 1895 in LaVara, Colorado, to William Henry and Melissa Jane Guymon Jewkes. Married Charles H. Blackham, July 11, 1913, in Price, Utah. He died November 28, 1919. Married W. Welzie Wayman, August, 1920, in Castle Dale, Utah. He died February 26, 1966. A devoted member of the LDS Church, Hazel was active in Primary and Relief Society, serving actively in the Castle Dale Ward for more than 40 years. She loved to cook and bake. She catered banquets and parties as well as supervised the school lunch program in her community.Survived by children and spouses, Ray and Eleanor Blackham and Leah Jacob, all of Castle Dale; Arilla Kulow, Kennilworth; Lorine and Rod Smith, Logan; Von and Barbara Wayman, Price; 18 grandchildren; 82 great-grandchildren. Preceded in death by two husbands, two sons, one stepson, four sons-in-law, four brothers, three sisters and her parents.

Funeral Services will be Monday, July 9, 11 a.m., in the Castle Dale Stake Center. Family and friends may call at Fausett Mortuary in Castle Dale, Sunday, July 8, 7-9 p.m. and at the Stake Center one hour prior to services. Interment: Orangeville City Cemetery.
