A Salt Lake County man was charged Monday with 12 felony counts of income tax evasion.

Utah State Tax Commission records show the 55-year-old Mill-creek man has not filed a personal income tax return since 1984, according to charges filed in 3rd District Court.W-2 forms show that between 1990 and 1995, the man's employer paid the man amounts that required him to file proper tax returns, court documents say. The man also has been issued three separate court orders to file tax returns, but has failed to do so.

In 1993, the man changed his number of exemptions from two to 10 on a W-4 form, which reduced the amount of taxes withheld from his paychecks, court documents say. Last January, he changed the forms to reflect exempt status.

The man has been charged with six third-degree felony counts of failing to make a proper tax return and six second-degree felony counts of willful evasion of income tax.
