Dr. John Blake Evans, 45, of Lehi, died June 4, 1996 at Utah Valley Hospital in Provo of cardiac arrest.

He was born May 14, 1951 in American Fork, Utah, son of Dean and Irene Binns Evans. Married Donalee M. Jones, Oct. 7, 1976 in the Provo LDS Temple. Blake graduated from Lehi High School, Utah State University, and Washington University School of Dental Medicine. He has had a private dental practice in American Fork for the past 20 years.He was past president of the American Fork Chamber of Commerce, past president of the American Fork Lions' Club, and past district governor of the State Lions' Club. For eight years he served on the Alpine School District Board of Education and served as President of the Board from 1991-1992.

Dr. Evans was a member of the American Fork Hospital Dental Staff and served as Director of Dental Services at the Utah State Developmental Center. He was a member of the Utah State Dental Medicaid Task Force.

He served in the LDS Church as ward clerk of the Lehi 13th Ward and at the time of his death was serving as a member of the Lehi North Stake High Council. His life was helping other people.

Survivors include his wife, Donalee, sons, Jared Dean Evans, Brock Don Evans; and daughter, Corinne Evans, all of Lehi; sister and brothers, Loraine (Wayne) Carlton, Aaron Dean (Mary Ellen) Evans, all of Lehi; Dr. Neal Binn (Barbara) Evans, Highland. He was preceded in death by his parents.

Funeral services will be held Friday, June 7, 1996 at 11 a.m. in the Lehi, Utah Stake Center, 200 No. Center Street, Lehi. Friends may call at the Wing Mortuary, 118 E. Main, Lehi, Thursday evening from 6-8 p.m. and also at the mortuary Friday morning from 9:30-10:30 a.m. Interment will be in the Lehi City Cemetery.
