Death: Myrtle C. Cook

Myrtle Collins Cook, age 65, died October 26, 1997 at home from heart failure.

She was born August 5, 1932 in Roosevelt to T. Golden and Florell Marx Collins. She married Orlan Cook, April 21, 1958 in the Salt Lake LDS Temple. She was a graduate of Utah State University, she taught at Todd School for 30 years, was Teacher of the Year in the Uintah School District, 1991 and one of the first 10 recipients of the Huntsman Excellence in Education, awarded by the governor in 1993.She was an active member of the LDS church, serving in many positions, served a mission to Brazil.

She is survived by her husband, Orlan of Ballard; sons, daughters and spouses: Judy (Lynn) Cornia, Riverton, WY; Peggy (Tracy) Killian; Roosevelt; Brent (Toni) Cook, and Grant Cook, all of Ballard; nine grandchildren; brothers and sisters, Boyd Collins, Clyde Collins, Mrs. Earl (Mary) Taylor, Allen Collins, Mrs. Ricky (Gaye) Berg, Mrs. Duane (Kaye) Frisby, Thomas Golden Collins, Jr.

Funeral services held Wednesday, 1 p.m. at the Ballard LDS Church. Friends may call at the Hullinger Olpin Mortuary Tuesday from 6-8 p.m. and Wed. at the church from 11:30 a.m. - 12:45 p.m. Interment, Roosevelt Memorial Park.
