A former University Hospital nurse was charged Monday with 15 counts of making a fraudulent narcotics report, a second-degree felony.

Michael R. Jablonski, 34, Murray, told detectives that he diverted pain medications to himself while working at the hospital during December 1996 and January 1997, according to charges filed in 3rd District Court.A narcotics report for Jan. 29-30, reviewed by a University of Utah police detective, shows the nurse dispensed Percocet a total of 15 times to three patients, the charges state.

But charts and screenings show one patient received just two Percocet, and two others never received the drug, prosecutors say.
