Our beloved and devoted daughter, wife, mother, grandmother and friend died July 11, 1998 at Cottonwood Hospital at the age of 55.
Dianne was the only child born to Melvin C. and Madge Poulson Hamm. She married John Arthur Gafa February 26, 1965 to whom she had and raised five children with.Her survivors include her mother; husband; her children, Dennis Gafa, Keith (Carrie) Gafa, Lisa Gafa and her companion Mike Stam; Johnny Gafa and his fiance Joanne Brice; Susan Gafa and her fiance Rick Mietchen; her grandchildren, Melanie, Amy, Audrey, Mallorie, Kyle, Nicholas, Ashly, Wade, Tyler, Tasha, Amanda, Scotty, Michael, Anthony, Sean, Jessica, Jennifer, Jamie, Justin and many other children who called her Grandma. Preceded in death by her father.
She was born in Mt. Pleasant, Utah and grew up in Salt Lake City. She attended Granite High School and was a member of the LDS Church. She had a love for the great outdoors like fishing, hunting, and especially the great mountains of Birdseye. She devoted her life to being a homemaker an taking pride in her family, extended family, and friends.
The family wants to express their love and thanks to Mom's very best friend to whom she grew up with, Dana Kay Wellington. We also send thanks to the staff of the Emergency Room and Intensive Care Unit at Cottonwood Hospital.
She touched the lives of every one who knew her and will be deeply missed by all. She is finally at rest, being reunited with her father.
Funeral services Thursday, July 16, 11 a.m., Goff Mortuary, 8090 So. State, where friends may call Wed., 6-8 p.m and Thursday one hour before the service. Interment, Mt. Pleasant Cemetery, at approximately 2:30 p.m. "GONE FISHING"
