PLEASANT GROVE — A five-car collision on I-15 killed a man Monday morning.

Killed was:

Patrick Hoffman, 40, Sandy.

Utah Highway Patrol officials say Hoffman lost control of his Toyota Camry around 8 a.m. while heading south on I-15 and slid into the northbound lane.

Hoffman's car was rolling by the time it entered the northbound lane where it was struck on the passenger side by a Chevrolet Suburban. Three other cars then collided with the wreckage.

Hoffman was wearing a seat belt but died at Utah Valley Medical Regional Center at 9:10 a.m. of head trauma. No one else was injured.

UHP troopers have not determined why Hoffman lost control of his vehicle.
