Roosevelt man dies in accident on U.S. 40

ROOSEVELT — A 74-year-old man died Wednesday after crashing his minivan into a tractor-trailer truck.

The Utah Highway Patrol said Tom Willeby of Roosevelt was driving east on U.S. 40 at about 12:30 p.m. when he fell asleep and crossed the center line. Willeby's Dodge Caravan slammed into a trailer loaded with diesel fuel being hauled by 38-year-old Troy Turner of Layton. None of the diesel fuel was spilled in the crash.

Roosevelt firefighters extricated Willeby from his van. He was taken to Uintah Basin Medical Center, where he died of his injuries.

Troopers said a third car, driven by 41-year-old Roosevelt resident Chris Black, suffered windshield damage caused by flying debris from the crash. Neither Turner nor Black was injured.

The crash shut down all but one lane of traffic on U.S. 40 for about an hour.
