Beloved son, brother, grandson, nephew, cousin and friend, Andrew Don Cook, age 12, died Tuesday, October 1, 1991 at the Mountain View Hospital in Payson.

He was born May 19, 1979 in Provo, the son of R. Don Cook and Kim Anne Berrett Cook. He was currently attending the Spanish Fork Intermediate School in the 7th grade. He was a member of the Church of Jesus Christ of Latter-day Saints, and a Deacon in the Spanish Fork 19th Ward. He enjoyed art work, family activities, playing sports, fishing, and the outdoors. he also enjoyed music and playing the violin and piano. He participated in choirs and plays. He has lived in Orem; Lawrence, Kansas; Lubbock, Tx.; Birmingham, Mich.; and Spanish Fork, for the past two months.Survivors include his parents; two sisters,

Christina Kim Cook, age 9; and Nicole Alicia Cook, age 2, all of Spanish Fork; grandparents, Robert and ARdis Cook, Woodland Hills; Bernell and Carolynne Berrett, and Larry Warren, all Salt Lake City; great-grandparents, Katherine B. Cook, Orem; Nellie Butterfield, Riverton; Ray and Ruth Cecil, Dean Warren, Deon Reese, all of Salt Lake City; numerous aunts, uncles, and cousins.

Funeral services will be held on Saturday, 1 p.m., in the Spanish Fork 19th LDS Ward Chapel, 360 North 650 West. Friends may call at the Walker Mortuary, 187 South Main, Spanish Fork, Friday evening, 6-8 p.m. or Saturday, at the ward Relief Society room, one hour prior to the services.
