Thomas Daniel Dwyer, M.D., 46, of Waialua, Hawaii, formerly of Salt Lake City, passed away June 7, 1995 at the Unversity of Utah Medical Center, following a weekend water-skiing trip to Lake Powell.

Dr. Dwyer was born in Elgin, Illinois, to Frank and Marvel Dwyer. He was one of seven children. He was a graduate of the University of Wisconsin and received his medical degree and specialty training in emergency medicine at Northwestern University in Evanston, Illinois. During his career he practiced in California, Utah and Hawaii. He was most recently associated with the Waianae Coast Comprehensive Health Center as a senior staff physician in the Emergency Department. The center is dedicated to providing health care for native Hawaiians on the island of Oahu. He also developed and was Medical Director of the Center's Occupational Medicine Program.Dr. Dwyer divided his time between practicing medicine and enjoying outdoor activities such as sky diving, swimming and water skiing. He also enjoyed restoring antique automobiles and motorcycles.

He is survived by his children, Samantha, age 14, Jamie, age 12, Charles, age 7, and his partner, Susan Hunt; his father, Frank Dwyer, Elgin, Illinois; his sister, Gloria Dwyer-Cohn, Hollywood, Florida; five brothers, Dr. William Dwyer, St. Charles, Illinois; Donald Dwyer, Big Fork, Montana; James Dwyer, Round Lake Beach, Illinois; Steven Dwyer, Salt Lake City, Utah; John Dwyer, Elgin, Illinois; and half-brother, Nels Nickelson, Elgin, Illinois.

He was deeply loved by his family and friends, including three nieces, seven nephews, one great-niece and one great-nephew. He was preceded in death by his mother, Marvel Svendsen-Dwyer.

In lieu of flowers, please send any contributions to the Thomas D. Dwyer memorial fund, in care of Steven Dwyer at 2087 South Belair Dr., Salt Lake City, Utah 84109. This fund is to be used to further benefit the Waianae Coast Comprehensive Health Center, which is dedicated to providing health care for native Hawaiians on the island of Oahu, and its Occupational Medicine Program.

Memorial services will be held at Larkin Mortuary, 260 East So. Temple, Salt Lake City, Utah at 6:00 p.m. Friday, June 9, 1995.
