Including reasons for leaving a former job almost always sends a negative message to a hiring manager. Save this information for the interview. If you must give an excuse, make it interesting:

"REASON FOR LEAVING: I blew the whistle and management blew its top."

"REASON FOR LEAVING: "Hours too long, boss' temper too short."
