Loving wife, mother, grandmother and great-grandmother Sally Chiazzese passed away October 25, 2001 at the age of 79.

Born May 19, 1922 in Falconer, New York to Thomas and Grace Polizzi. Married Joseph Chiazzese Sept. 1, 1941 at Our Lady of Loretta Catholic Church in Falconer, NY. All that we are and all we have become is in someway a tribute to her. Wife, mother, grandmother and friend; Sally's love will always remain the most important influence in our lives. Goodbye for now, we will miss you more than words can ever say, we love you.

Survived by her husband Joseph, daughter Rosemary (Gerald) Vincent, son Lenny (Linda). Grandchildren, Christine Vincent, Jerry (Beth) Vincent, Stacey (Cory) Roper, Tony Vincent, Joe (Bridget), Jon (fiancee Lindsay Bevans), Michael and Christopher Chiazzese. Five great-granddaughters, one great-grandson. Brother Russell (Ann) Polizzi. Sister Josephine (Dick) Campbell, New York.

The family would like to thank the staff at Millcreek Retirement Center for there loving care.

A funeral Mass will be celebrated Monday Oct. 29, 2001 11 a.m. at St. Ann's Catholic Church 450 E. 2100 S. where friends may call Sunday October 28, from 5-7 p.m. Rosary will be recited at 7 p.m. Entombment Mt. Calvary Cemetery 4th Ave. and "T" Street Funeral Director's Neil O'Donnell and Sons Mortuary.
