Rosemary "Mama" Cosby, founder of the Faith Temple Pentecostal Church, died of "heart failure due to a combination of coronary artery atherosclerosis and aortic valve insufficiency," according to a 1999 autopsy report. A story on Wednesday did not specify exact cause of death.
