DUCHESNE — Officials have identified a Spanish Fork man who died Thursday after slipping and falling into a reservoir in Duchesne County, officials said.

Peter Tomadakis, 79, was at the shore at Starvation Reservoir in Fred Hayes State Park with a friend. While the pair was walking along the shore near the side of a bridge, Tomadakis slipped and fell about 3:40 p.m. Thursday, rolling several feet down a bank and into the water, Utah State Parks officials said in a statement.

Tomadakis' fishing companion was unable to save him and called 911. The Duchesne County Sheriff's Office and a park ranger responded to the scene and recovered Tomadakis, officials said. He was pronounced dead at the scene.
