SALT LAKE CITY — Two former PGA Tour winners and one of the top amateurs ever to come out of Utah will be inducted into the Utah Golf Hall of Fame this spring.

St. George native Jay Don Blake will join Provo resident Dan Forsman and Taylorsville's Todd Barker at a ceremony planned for May 11th at Hidden Valley Country Club.

The selections were made last month by a committee consisting of representatives of the Utah Golf Association, the Utah Section PGA, the media and Hall of Fame members.

Blake is the first Utah native to play regularly on the PGA Tour, and is currently playing on the PGA Champions Tour. He played college golf at Utah State University and won the NCAA medalist honors in 1980 and was runnerup the following year when he was named NCAA Player of the Year. On the PGA Tour he won the Shearson Lehman Brothers Open in 1991 and has career earnings of more than $6.5 million.

Forsman has won five times on the PGA Tour, twice on the PGA Champions Tour, and has career earnings of nearly $9 million. He has lived in Utah since his collegiate days at Arizona State and is a member of Riverside Country Club and an active supporter of Utah golf activities.

The 56-year-old Barker, who is part owner and superintendent of Fore Lakes Golf Course, has been one of Utah's top amateurs since the 1970s. He was named UGA Player of the Year five times, in 1986, 1990, 1992, 1993, and 1997. He won the Utah State Amateur championship in 1997, the Salt Lake City Amateur five times and has won the Golf Course Superintendents National championship twice.

The Utah Golf Hall of Fame was established in 1991 and currently has 30 inductees.
