PARAGONAH, Iron County — A teenage girl and an elderly woman from California died Sunday in a crash in Iron County, troopers said.

At 7:25 a.m., a family of four was driving north on I-15 in a Honda Odyssey van in Paragonah when the vehicle’s rear right tire “experienced a failure,” causing the driver to lose control, the Utah Highway Patrol said.

The van hit the median and rolled twice, according to troopers. Zahara Ismael, 14, and Hamdiyan Salman, 74, both of San Diego, were riding in the second row and were not wearing seat belts, the UHP said. They were ejected when the van rolled.

Zahara died on scene. Salman was taken to a hospital, where she died.

The driver and front seat passenger were wearing seat belts and suffered injuries that did not appear life-threatening. They were both taken to local hospitals.

“An inspection of the vehicle was completed on scene and revealed the vehicle had multiple equipment violations and was not safe to drive on the roadway,” troopers said.
