A pharmacist was fined $1,500 for supplying "Diff'rent Strokes" actress Dana Plato with more than a year's supply of Valium in three weeks.

Meanwhile, Plato, 27, who is on probation for robbing a video store, pleaded guilty Wednesday to forging prescriptions. She is to be sentenced June 9.The state Board of Pharmacy cited Charles Kotzan, 63, of Clinic Pharmacy, for not following professional standards when he supplied the drugs last year. He also received a year's probation.
